On April 28, 2021, a Northwest driver failed to lift the catwalk on a sulphur loading rack when
pulling forward. This incident has been deemed preventable and has reset the Days Since
incident counter for Northern BC.

As you can see from the image below, the driver did extensive damage to the rack and the initial
repair estimate is $25,000. The rack is out of commission for the next month and a temporary
measure has been put in place to ensure we can continue to load.

The main contributing factors to the incident were:
● Failure to look up during a circle check of the unit
● Failure to follow safe work practices
● Distraction and inattention

Our driver was distracted by an event that took his attention away from the loading of the trailers
and he missed a step in the process, creating a dangerous and costly situation.
